In a medium pot, combine milk, salt, and sugar. Stir constantly over medium-high heat until the sugar's gone.

Turn the heat down for a soft simmer. Gently stir the pot's bottom sometimes but skip the whisking. Let it go for 30-45 minutes.

Once the liquid's reduced by half and darkened like syrup, stir in the vanilla.

If there's foam or bits floating around, strain it. Leave it to cool completely after that.

Pop it into a sealed container, then store it in the fridge for up to a month. Briefly heat it up if it thickens too much.
