In your slow cooker, stir cocoa powder into 2 cups heavy cream until it's smooth and lump-free.

Mix in the rest of the cream, milk, sweetened condensed milk, vanilla, and the chopped chocolate.

Set the slow cooker on low heat, cover, and let it cook for around 3 hours, giving it a stir every 20 minutes so it doesn’t burn.

Turn to the warm setting once it's cooked. Thin it out with a splash of milk if it's too thick. You can leave it warming for up to 3 hours.

Pour into cups and toss on marshmallows, candy canes, or whipped cream as toppings.
